Not submittedRead it the way you would on the monitor. Turn on Calipers, stage each interval in the measurement panel and press Apply — PR, QRS, QT and R–R (Rate follows from R–R). Then pick the rhythm and submit. Intervals are graded with a little leeway: ±40 ms (one small box) or ±10% of the true value, whichever is larger; Rate gets ±5 bpm. Anything you don't Apply is marked “Not measured” — it doesn't count against you.
Practice questions
These open the actual NCLEX bank questions — the answers, gold-standard rationales and open-textbook sources live in the NCLEX Trainer, so what you practise here is the real bank, not a copy.

Normal sinus rhythm — the regular rhythm
Every rhythm in the table below is a variation on this one, so it comes first. This is the strip all the others are compared against: the sinus node fires, the impulse spreads across the atria (the P wave), crosses the AV node on schedule (the PR interval), and races through the ventricles (a narrow QRS) — beat after identical beat.
- Rate 60–100/min — and regular: every R–R interval the same.
- One upright P wave before every QRS — one P : one QRS, every time.
- PR interval 120–200 ms (3–5 small boxes), constant on every beat.
- QRS narrow — under 120 ms (under 3 small boxes).
The strip above is drawn to sit inside every one of those limits. When a practice strip breaks one of them, the broken rule is usually the diagnosis.
Drawn at the standard settings used across this app: paper speed 25 mm/s, calibration 10 mm/mV. Time runs left to right — count boxes horizontally for intervals (PR, QRS, QT) and vertically for voltage. The PR interval is measured from the start of the P wave to the start of the QRS; the QT interval from the start of the QRS to the end of the T wave.
